The case, who discribed here is a 66-year-old man with one year history of asymptomatic, keratotic papules with a linear distribution on the skin of his right palm near the wrist. On histopathological examination cornoid lamella-like parakeratotic columns above eccrine sweat ducts were observed. The acrosyringium was also dilated. He had been followed up for predominantly sensory polyneuropathy by neurologist from 6 years before. Endocrinologic evaluation revealed hyperthyroidism. In this article, we present a new case of Porokeratotic Eccrine Ostial and Dermal Duct Nevus (PEODDN) where the palmar skin was the primary site of presentation. The present case is consistent with the tarda variant of PEODDN, as the onset of lesions was around the age of 65 years.

Acquired ichthyosis is a known paraneoplastic sign of lymphoproliferative malignancies with nonspecific histopathologic findings revealing no implication of the underlying neoplasm. However, ichthyosiform eruption is considered as a specific manifestation of mycosis fungoides (MF), i.e., ichthyosiform MF. There are a few reports of ichthyotic manifestation of MF, but only one report of this presentation in Parapsoriasis. We present a case of ichthyosiform Parapsoriasis in a 22-year-old woman with hyperpigmented ichthyosiform scaly patches on her trunk and extremities. The histopathologic findings were hyperkeratosis, parakeratosis and acanthosis with scattered lymphocyte infiltration, also could be seen in Parapsoriasis. The patient responded well to oral photochemotherapy.

Background: Acne vulgaris is the most common disorder of pilosebaceous unit. Systemic antibiotics are known to be effective in its treatment. We performed this investigation to compare the efficacy of azithromycin with doxycycline in the treatment of acne vulgaris. Method: A twelve-week study was performed on 69 patients with moderate facial acne to compare the efficacy of oral azithromycin with oral doxycycline. Sixty patients completed the study. Patients in the treatment arm one were scheduled to receive 500 mg azithromycin once daily three times a week, and patients in the treatment arm two were instructed to use 100 mg doxycycline daily. All patients administered topical tretinoin cream every other night. Clinical assessment was made at baseline and then every 4 weeks. Result: There were statistically significant improvements in comedones and inflammatory lesion counts in both groups. Neither drug was shown to be more effective than the other. Conclusion: This study indicated that azithromycin had similar efficacy to doxycycline in reducing acne lesions.

Piebaldism is an autosomal dominant uncommon (<1 in 20,000) congenital pigmentary disorder. Depigmented patches are present since birth. They usually remain unchanged throughout life. Vitiligo is its closest differential diagnosis. We report a unique family in which these two dissimilar depigmentations, i.e. piebaldism and vitiligo (with nevus depigmentosus), were noted in two brothers. To the best of our knowledge, this is the first report of this presentation in the literature.

Background: An elevated homocysteine level is an independent risk factor for cardiovascular disorders. Psoriatic patients have an increased risk of cardiovascular diseases; In addition, hyperhomocysteinemia is a complication of methotrexate treatment. We undertook a study to evaluate the plasma levels of homocysteine, vitamin B12 and folate in patients with psoriasis before and after short-term low-dose methotrexate treatment. Methods: Twenty six patients with psoriasis were recruited. The plasma levels of homocysteine, vitamin B12 and folate were evaluated before and 8 weeks after methotrexate therapy (in the peak of methotrexate effect). Results: No significant difference was found between the plasma profile of homocysteine, vitamin B12 and folate before and after methotrexate treatment. Conclusion: In the short-term treatment of psoriasis, methotrexate does not increase homocysteine level.

Introduction: Psoriasis is an inflammatory skin disease in which abnormal individual immune reactivity plays an important role. Eselectin and P-selectin are adhesion molecules expressed on vascular endothelial cells in several inflammatory skin diseases, including psoriasis. The aim of the present study was to describe selected immunological changes, concerning adhesion molecules status (sEselectin, sP-selectin), in psoriasis and also their correlation with disease activity. Method: Serum levels of soluble E-selectin and soluble P-selectin were measured by enzyme-linked immunosorbent assay in 58 patients with psoriasis and 24 healthy control subjects. The relationships between these adhesion molecules and the Psoriasis Areaand Severity Index score were investigated. Result: There was a significant correlation between the serum soluble E-selectin levels and Psoriasis Area and Severity Index score. Conclusion: Soluble E-selectin serum levels correlate with the extent of psoriatic lesions and could be used as marker of the disease activity in psoriatic patients but this finding needs further modification.

Background: To our knowledge, there is a hypothesis regardingthe association of vitiligo with other autoimmune disorders.Organ-specific autoantibodies are found more frequently in theserum of vitiligo patients. Recent studies have demonstratedthe role of alterations in serum transforming growth factor-beta(TGF-beta) released from regulatory T cells in the pathogenesisof depigmentation observed in vitiligo. It has been shown that incomparison with patients without autoimmune diseases, serumTGF-beta levels increase in patients whose vitiligo is associatedwith autoimmune diseases. We hypothesized a relationshipbetween serum TGF-beta levels and organ-specific autoantibodiesthat could predict other autoimmune diseases in vitiligo patients.Method: Forty-five patients with a mean age of 35.96±13.34years who had stable vitiligo since 1 year ago and involvementof up to 30% body surface area were enrolled. Organ-specificautoantibodies (ANA, anti mitochondrial Ab, anti TPO (antithyroid microsomal Ab), anti parietal cell Ab, anti thyroglobulinAb) and serum TGF-beta level were evaluated.Result: Twenty-three patients (51.11%) had at least one positiveorgan-specific autoantibody. Anti TPO in 16 patients, antithyroglobulin Ab in 9 patients, anti parietal cell Ab and ANAeach in 5 patients, and anti mitochondrial Ab in 4 patients werepositive. Mean serum TGF-beta level was 105.82±30.33; there wasno significant difference in serum TGF-b level between patientswith and without positive organ-specific autoantibody (P=0.26).Conclusion: Although another study showed the relationshipbetween serum TGF-beta levels and autoimmune disordersin vitiligo patients, we did not find a significant difference inserum TGF-beta levels in these patients regarding the positivityof organ-specific antibodies. It may be due to the fact that ourpatients had autoantibodies without clinical autoimmune diseaseexcept vitiligo.

Nowadays, the coronavirus disease 2019 (COVID-19) pandemic has become a global health problem and, given the high mortality and transmission rate of this infection and the lack of approved or specific treatments for it so far, the exploitation of the potentials of existing drugs is of paramount importance, hence, we aimed to investigate all possible COVID-19-related mechanisms of systemic retinoids. We systematically searched all related evidence in various data bases about the effect of retinoic acids on respiratory failure as the most important COVID-19-related complication and the other important potentials of this drug to affect COVID pathomechanism. Considering the various behaviors and presentations of this virus in different people, paying attention to the underlying diseases, physical conditions, genetics, and medications that cause these differences may be helpful in reducing the mortality and morbidity of the disease. In the field of dermatology, retinoids are a group of drugs that have controversial evidence regarding their effects when administered to patients with COVID-19. On one hand, these drugs offer immunomodulatory, immuneenhancing, antiviral, and antiangiogenesis activity, and can stimulate the differentiation and repair of epithelial cells. On the other hand, they can increase inflammation and cause the fragility of oral and nasal mucosae. When prescribe systemic retinoids, all of these aspects should be considered in all fields of the medicine and dermatology is not an exception.

The aim of this study wasto evaluate the antimicrobial activity of nettle leaves (Urticadioica), colocynths fruits (Citrullus colocynthis), green tea (Camelliasinensis), burdock root (Arctium lappa) extracts and ketoconazoleagainst 26 isolates of Malassezia spp., isolated from patients withMalassezia infections.Method: Twenty-six (26) Malassezia spp. identified by RFLPPCR,were isolated from patients with Malassezia infections. Theantifungal activity was evaluated by micro broth dilution assay.Result: M. globosa (50%), M. furfur (46%) and M. slooffiae (4%)were the isolated strains. There was no significant differencebetween the minimal inhibitory concentration (MIC) values forcolocynth extracts and ketoconazole. Also, the antimicrobialactivity of green tea and burdock extracts against Malassezia spp.was not significantly different (P>0.05). The minimal fungicidalconcentration (MFC) values for green tea, colocynth extracts andketoconazole against Malassezia spp. were the same (P>0.05).Conclusion: Further clinical studies are required to determinethe efficacy of C. colocynthis ethanol extract, in the treatment ofMalassezia skin disorders.

Epidermal nevi are cutaneous hamartomas developing from ectoderm and one of their types is verrucous epidermal nevus. During the neonatal period, they are pink or slightly pigmented and velvety. Later, they darken and the surface become more warty. When lesions are distributed on one side of body, it is termed nevus unis lateralis. Lymphangioma is a lymphatic malformation which manifests with fluid- filled vesicles bulged on the skin surface.We present a 30- year – old man with pigmented, hyperkeratotic plaques on right side of his body associated with clear vesicles predominant at right side of his natal cleft. The histopathologic report of above lesions were epidermal nevus and lymphangioma, respectively.

Background: In addition to humoral immunity associated with anti-desmoglein antibodies, cellular immunity and mediators including cytokines are involved in the pathogenesis of pemphigus vulgaris. In this study we evaluated the level of IL-2, IFN-γ, TNF-α, IL-4, and IL-10 in the sera of patients with pemphigus vulgaris before and after treatment.Methods: A total number of 71 new patients with pemphigus vulgaris were included in the study. The above mentioned cytokines were measured in patients with a mild disease (20 bullae or less) and a severe disease (60 bullae or more) using ELISA method before and 4 weeks after treatment with 2 mg/kg/day prednisolone and 2mg/kg/day azathioprine. We also measured IL-4 and IL-10 in 69 mildly and 70 severely affected patients only at the beginning of the study. All patients had muco-cutaneous phenotype. Patients with a mild disease had mild mucosal involvement and patients with a severe disease had moderate to severe mucosal involvement. Serum levels of IL-2 and IFN-γ were also measured in 27 normal controls.Results: In the total study population, the level of IL-2 decreased from 103.9 pg/ml to 82.79 pg/ml after treatment (p=0.05). Comparing cytokines between 2 groups (severe and mild), the level of IL-2 before treatment showed significantly lower figures in severe patients (147.27 versus 67.38, p=0.04). On the other hand, IFN-γ after treatment was significantly higher in severe patients (0.75 versus 0.42, p=0.04).Conclusion: Mean level of IL-2 is lower in severe pemphigus vulgaris patients than that of mild disease. This finding indicates that, in pemphigus vulgaris, IL-2 level negatively correlates with the severity of the disease and widespread underlying autoimmune process. The data also suggests that the level of IFN-γ directly correlates with the severity of the disease.

Background: Lichen planus (LP) is a chronic inflammatory skin condition which leads to changes in lipid metabolism. It may cause chronic atherosclerosis and metabolic syndrome. Theobjective of this study is to compare lipid levels of patients with LP to healthy controls. Methods: This case-control study recruited 100 total participants, 50 (25 male and 25 female) patients with LP and 50 healthy controls admitted to the Dermatology Clinic of Imam Hospital, Ahvaz, Iran. Serum triglyceride, total cholesterol, HDL cholesterol, and LDL cholesterol levels were measured in samples drawn after a 12-hour fasting period. Results: Patients with LP had significantly higher triglycerides (147.7 vs. 118.4 mg/dl, P<0.05), total cholesterol (188 vs. 173 mg/ dl, P<0.05), LDL (122 vs. 106 mg/dl, P<0.05), and lower HDL (49 vs. 57 mg/dl, P<0.05) levels compared to controls. Conclusion: This study showed an association between LP and dyslipidemia. Screening of serum lipids in patients with LP might be valuable to prevent cardiovascular diseases.

Basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) are the most frequent skin cancers. The etiology of these tumors is multifactorial: Environmental and exogenous factors, genotype and phenotype factors. The development of malignant neoplasms at sites of previous dermal scars is an uncommon but well recognized phenomenon, and since almost all of them were developed on exposed areas it has been hypothesized that the accompanying atrophy and decreased vascularity and adnexal structures in areas of scarring may render the affected tissues more susceptible to the effects of environmental factors, eg. ultraviolet light. We report four cases of BCC and SCC on old leishmania scars on face, and recommend that any changes on scars must be evaluated clinically and pathologically.

Background: Laser treatment is a rapid, non-invasive procedure for long-term hair removal. Lasers operate based on the principle of selective photothermolysis and target melanin in the hair follicle. We intend to determine the level of hirsute patient satisfaction with the alexandrite laser hair removal procedure.Methods: In this cross-sectional study 82 hirsute patients with unwanted hair on their faces were enrolled. The patients received treatment with a 755 nm alexandrite laser (GentleLASE, Candela Corporation, USA). We documented their level of satisfaction with this laser in terms of hair removal. Laser therapy, electrolysis, and depilation were compared.Results: Patients ≥30 years old expressed greater satisfaction compared to those <30 years of age (P=0.03). Patients with medically related hirsutism had the same satisfaction as the idiopathic hirsute group (P=0.81). Patients rated laser hair removal as 4.12 compared to depilation and 3.96 compared to electrolysis. From total patients, 60.97% would recommend laser treatment to other hirsute patients. There were 79.3% of patients who expressed satisfaction with alexandrite laser hair removal.Conclusions: Most patients expressed satisfaction with the alexandrite laser hair removal procedure. Age played a significant role in patient satisfaction.

Background: Treatment of vitiligo remains an attractive topic and several therapies with varying degrees of success have been used. The aim of this study was to find out whether the combination of topical calcipotriol and excimer laser increases the efficacy of therapy compared to excimer laser alone. Methods: Twenty eight patients in two groups were treated with 308nm excimer laser alone (14 patients) and the combination of excimer laser–calcipotriol (14 patients). After 16 sessions, response rate, side-effects and compliance were assessed. Results: Repigmentation rate more than 50% was significantly higher in the laser- calcipotriol group. No differences were seen in complication rate between the two groups. Conclusion: Combination of calcipotriol and 308nm excimer laser might enhance the response rate without increasing treatment complications.

Background: Vitiligo is a common disease. Its etiology is not known, but 30% of patients have a positive family history. Studies have been done on the influence of ABO and Rh blood groups on susceptibility to some diseases. Objective: The purpose of this case-control study was to evaluate the association of ABO and Rh blood groups with vitiligo. Patients and Methods: This study was performed on 200 vitiligo patients referred to skin clinics of Razi Hospital. The blood groups of patients were determined and compared with 200 control subjects and the general blood group distribution in Iran (Published by Iran Blood Transfer Organization). Results: There was no specific association established between vitiligo and ABO and Rh blood groups. Blood group B showed the most difference but still it was not significant (P>0.05). Conclusion: There was no association with the ABO and Rh blood group in vitiligo patients.

Background: Seborrheic dermatitis (SD) is an inflammatory disease affecting seborrheic areas of the skin. Scalp SD varies from mild, manifesting as dandruff, to erythematous lesions with greasy scales. SD negatively affects patients’ quality of life.Methods: This is a cross-sectional study investigating the relationship between quality of life and disease severity in patients with dandruff and scalp SD at the Dermatology and Venereology Outpatient Clinic, of Dr. Cipto Mangunkusumo General Hospital. SD severity was assessed by the modified Seborrheic Dermatitis Area and Severity Index (SDASI), and quality of life was assessed by the Indonesian version of the Dermatology Life Quality Index (DLQI).Results: A total of 96 subjects with scalp SD were recruited, comprising 86 subjects with mild SD and 10 subjects with moderate SD. DLQI scoring revealed 5 subjects (5%) felt no effect of SD at all on their lives, 10 subjects (10%) felt small effect, 38 subjects (40%) felt moderate effect, 32 subjects (33%) felt very large effect, and 2 subjects (2%) felt extremely large effect on their lives. There is no significant difference between SD severity with the average DLQI score in the mild SD group (8.73 ± 4.9) and the moderate SD group (9.30 ± 6.13), P=0.737. Furthermore, no significant difference was found between the DLQI scores in the male (9.86 ± 4.7) and female (8.18 ± 5.13) groups, P=0.116. Spearman’s correlation analysis revealed no significant relationship between age and DLQI scores (r=-0,104, P=0,313).Conclusions: Our study concludes that SD negatively affects the quality of life.

Background: Acne vulgaris is a common skin disorder that affects 79% to 95% of the young population. Some studies have focused on the role of overweight and obesity in initiation and exacerbation of acne. This study investigated the relationship between BMI and the severity of acne. Method: This cross-sectional study was conducted on 107 acne patients. One form was completed for each patient with regard to demographic characteristics including age, sex, weight, height, BMI, and also the severity of acne based on the Global Acne Severity Scale (GEA Scale), duration of the disease, type of medication, smoking history, duration of exposure to sunlight and intake of dairy products and chocolate. Result: In this study, 93 patients (86.9%) were female and 14 patients (13.1%) were male, with a mean age of 22.5 years. The mean BMI of the participants was 23.36 kg/m2 . With regard to acne severity groups, the moderate acne group had the highest frequency in total (37.4%). The highest mean BMI was seen in the very severe acne group (24.4 kg/m2 ), and the lowest mean BMI was observed in the severe acne group (21.86 kg/m2 ). There was no significant correlation between the acne severity and BMI (p=0.806).The mean disease duration was 4.13 years, which was not significantly associated with BMI. There was no significant association between the type of medical therapy and BMI. Conclusion: The prevalence of acne with severity more than moderate was much lower in patients who were underweight (BMI less than 18.5). However, this study showed no statistically significant difference in BMI among people with different severities of acne. Population-based studies, as well as age restriction of the participants, can lead to more accurate and reliable results in this regard.

Background: Some skin diseases cause severe stress in patients and relieving these stresses greatly helps to treat the underlying disease and to prevent the recrudescence of it. Objectives: To determine the frequency of depression in patients suffering from vitiligo. Patients and Methods: 120 patients with a clinical diagnosis of vitiligo referred to dermatologic clinics in Kerman from 1380 to 1381 were studied. The impression of vitiligo was made according to clinical interview and physical examination. After filling the questionnaire the patients were referred to psychiatric clinic to fill in the Hamilton questionnaire. Results: 120 patients included 74 women (61.6%) and 46 men (38.4%) with mean age of 23±13 years. 30.83% (37 patients) had no depressive disorder, 38.34% (46 patients) had depressed mood and 30.83% (37 patients) had major depressive disorder. From 37 patients with major depressive disorder, 27 patients (73%) were female and 10 patients (27%) were male (P<0.001). Conclusion: Any clinical intervention that reduces the extension and visibility of depigmentation will have psychologic benefit. It may be important to recognize and deal with the social and psychologic consequences of the disease.

Background and aim: Skin cancer is the most common human cancer. Ultraviolet radiation is now recognized as the most important risk factor for skin cancer development. Light skin colored individuals that spend more time outdoors in day are more susceptible to disease. In this study skin cancer situation in Yazd (located in a hot, dry and desert area) was reviewed.Materials and methods: In this existing data study all recorded cancers in pathology laboratory of University of Medical Sciences of Yazd between 1988-1996 were reviewed and skin cancers were studied.Results: 2360 patients with cancer were recorded. Skin cancer was the most common with 646 cases (27.3%). Bladder, stomach, breast and esophagus cancers were the other most common cancers. Among skin cancers basal cell carcinoma was the most common (77%). Other skin cancers included squamous cell carcinoma (17.8%), malignant melanoma (2.8%) and miscellaneous tumors (2.4%). Skin cancers were 1.5 times more common in males, and the majority of patients were in seventh and eighth decades of life. 91.5% of skin cancers were on face, head and neck.Conclusion: Skin cancer was the most common human cancer in Yazd. Considering skin type and intensity of sun in this area, more intensive preventive programs must be implemented.

Background: Pyogenic granuloma is a benign vascular proliferation that is often related to minor trauma by the patient. Objective: Evaluation of the efficacy of cryotherapy in the treatment of pyogenic granuloma. Patients and Methods: Hundred and forty-six patients with pyogenic granuloma were treated by cryosurgery using liquid nitrogen and cotton tip applicator in an open clinical trial. This study was carried out in Razi Hospital, Tehran University of Medical Sciences, in 1377. Patients were followed for at least 3 months after complete resolution. Results: Hundred and thirty-five patients completed the study. Complete resolution was achieved in all patients. Treatment sessions ranged from one to 4 (Mean 1.6±0.8) sessions. 88.2% of patients needed only 1 or 2 treatment sessions. Recurrence of lesions occurred in 3 patients in whom retreatment led to complete resolution. Conclusion: Cryotherapy can be regarded as a completely efficient method in treatment of pyogenic granuloma.

Background: Diabetes is an important metabolic disease with myriad manifestations and complications, affecting many people. Cutaneous infections impact numerous diabetic patients. Among different bacterial infections in diabetic patients, the infections caused by mycobacteria other than Mycobacterium tuberculosis (MOTT) eventuate in complications owing to the paucity of accurate detection methods.Methods: The articles reporting nontuberculous mycobacteria (NTM) in cutaneous infections of diabetic patients, published until the end of 2017 were assessed in the present research.Results: The organisms reported from cutaneous infections of diabetic patients are Mycobacterium chelonae, Mycobacterium abscessus, Mycobacterium immunogenum, Mycobacterium kansasii and Mycobacterium fortuitum.Conclusion: NTM infection, along with prolonged disease cycle, decelerates the healing process in diabetic patients. Considering NTM during infection diagnosis, along with other possible opportunistic bacteria, conduces to accelerating the treatment process. In most cases, clarithromycin and erythromycin have been reported as effective alternatives for the treatment of diabetic patients.

Background: Paederus dermatitis, an acute erythemato-vesiculo-bullous dermatitis contracted through contact with the secretions of the Paederus species, is frequently seen among the people living in the south-west part of our country during spring and early summer months. Objective: This study was conducted to elucidate the responsible arthropod in Khuzestan province and to study the clinical spectrum as well as the pathological findings at different stages of the disease. Patients and Methods: Within a period of about 3 months, from April to July 1999, 72 patients referred to our clinic with erythema, vesicles, bullae, pustules, superficial skin necrosis or combination of few or all of these findings clinically suggestive of Paederus dermatitis. Biopsy samples were obtained from 19 patients at different stages of the disease stained by HE and studied histologically. All arthropods submitted by the patients and their families were crushed on our skin to identify the causative species. Results: The dermatosis is contracted through contact with the secretions of a Coleoptera species called Paederus Ramtinus. The painless and non-pruritic reaction developed 2-3 days following contact as erythema and vesiculation, which may gradually extend to necrosis and bullae. The pathologic changes were also variable and ranged from epidermal spongiosis to complete epidermal and upper dermal necrosis. Conclusion: Paederus dermatosis developed as a result of contact with the secretions of the species Paederus Ramtinus if by any way crushed or brushed off over the skin. The dermatosis is benign in contrast to its acute course no sequela is seen except for post inflammatory hyperpigmentation.

Background: So far seven types of the genus malassezia have been known. Except for one kind, the rest are normal flora of human skin, which under certain circumstances, cause or trigger eleven diseases. One of the side effects of topical and systemic steroid is steroid acne. Objective: This study was conducted to evaluate the association of pityrosporum ovale (Malassezia) and steroid acne. Patients and Methods: Skin smears were prepared by comedone extractor from 20 patients with steroid acne and 12 acne patients without history of steroid usage, referred to dermatology clinic of Qaem Hospital and checked for the presence of malassezia. The reports of 2+ or more were considered as positive. The skin smears were positive in 80 percent of patients with steroid acne, and 66 percent of control group patients (P<0.05). 16 patients of steroid acne group responded well to antifungal drugs. Conclusion: Considering the clinical symptoms of steroid acne, positive skin smears and clear response of the patients to antifungal drugs, some of the steroid acne lesions are pityrosporum folliculitis.
